About

We’ve Connected Health and Community Information for a Healthier Texas

Healthconnect Texas is a 501(c)(3) nonprofit organization that helps partners across Texas connect information, strengthen coordination, and turn data into action. Today, we bring together the strengths of a health information exchange (HIE) and a community information exchange (CIE) so organizations can better understand and respond to the full picture of health.

Mission

To connect all venues of care – integrating different electronic health record systems so that providers can easily share medical information across the community.

Vision

To operate a sustainable health information exchange that coordinates care for the community, improves overall population health, enhances health and wellness initiatives, reduces healthcare costs, and advances opportunities for long-term research and disease prevention.

Who we are

Health outcomes are shaped by more than clinical care alone. They are also influenced by community conditions, social needs, public health realities, and the systems people rely on every day. Healthconnect Texas helps bridge those realities by connecting clinical insight with community context.

We work with healthcare organizations, community-based organizations, public health partners, researchers, and other stakeholders that need stronger information-sharing, better coordination, and clearer insight to serve Texans more effectively.

Our evolution

Healthconnect Texas has grown to support all Texans with more depth and breadth than ever before. Through our strategic unification with Patient Care Intervention Center (PCIC), we now bring together statewide clinical connectivity with community-based insight into the non-medical drivers of health. This unification expands our ability to support whole-person care, cross-sector coordination, and data-informed strategies that improve outcomes across systems.
